Alliance accepts most insurance carriers. Please be sure to bring your insurance card and photo ID. If your insurance requires you to list a Primary Care Provider (PCP) and Alliance is not listed with your carrier, you will need to call your insurance to make the change prior to your appointment. Most insurance companies list a phone number for member services on the back of your card.
If you are currently uninsured, the team at Alliance Community Healthcare will work with you to determine if you are eligible for Medicaid or one of several other programs, and help you apply.
As a Federally Qualified Health Center, Alliance Community Healthcare may be able to help patients who do not have health insurance plans cover a portion of the cost of care based upon their income. The amount of a discount a patient may be eligible for is based upon a sliding-fee scale, which takes into account household income and the number of people in one’s household. If you do not have a health insurance plan and are not eligible for Medicare or Medicaid, we ask that you bring the following information and documents with you on a first visit:
- Identification: photo ID, valid driver’s license
- Proof of income: current pay stub, unemployment stub, letter from your employer, letter of support, child support payment document or foster care allotment
- Proof of address: a telephone, cable or utility bill
For Alliance Community Healthcare’s Patients/members wishing to Pay at the time of service, Alliance has very affordable and reasonable fees. The finance team will work with you on a payment plan if needed or you can pay at the time of service directly with Cash, Credit/Debit Cards or Check.
